How to Treat Nasal Polyps in Children

nasal polyps in childrenIndividuals suffering from certain respiratory disease conditions that continue to recur, for eg., cystic fibrosis, have an inclination to develop nasal polyps.  Cystic fibrosis is more of a genetic problem that causes mucus to accumulate in the lungs.

Children suffering from cystic fibrosis or other respiratory conditions causing continuous inflammation, like asthma, allergic rhinitis, and chronic sinusitis are likely to develop nasal polyps; at least one-fourth of the children with cystic fibrosis will acquire nasal polyps.  Hence, even in the absence of its symptoms, cystic fibrosis should be suspected when children present with nasal polyps.

Nasal Polyps – How to identify ?

Nasal polyps arise from a chronically inflamed state of the nasal and sinus mucosal linings that become edematous.  These are sac-like lesions that generally occur in both nostrils, and very rarely can grow in only one nostril.  (Polyps growing only in one nostril will need immediate medical attention since these can be cancerous growths).  The polyps can grow anywhere in the sinuses, but usually occur in the nasal cavity, present in the middle meatus.  However, in children, polyps occur more commonly as an antrochoanal polyp that originates in the maxillary sinuses.

Symptoms

–       Obstruction of nasal passages

–       Continuous postnasal drip

–       Nasal speech

–       Mouth breathing and snoring

–       Tear-drop shaped fleshy protrusion in the nasal cavity, that may be single or in clusters

Diagnosis

First, a visual examination of the external nose areas is performed.  This is followed by a rhinoscopy and a flexible nasopharyngoscopy to diagnose the presence of antochoanal polyps, since these can prolapse into the nasopharynx.  A CT scan of the middle facial regions should also be obtained for a definitive diagnosis to be made, to arrive at a more appropriate treatment plan.

Nasal Polyps in Children  Treatment Methods

Medical Therapy

Decongestants do not usually work towards effectively shrinking the polyps, but may provide symptom relief.  Use of corticosteroids seems to be the only most effective manner by which nasal polyp growth can be well controlled medically.  Steroid sprays and drops sometimes shrink the size of nasal polyps along with providing symptom relief and can be safely used to treat children with nasal polyps.  Also, steroid injections injected directly into the polyps will successfully retard their further growth.  Though steroid tablets are also capable of shrinking nasal polyps, they are generally not prescribed for children, since its long term use can lead to severe health complications.

When symptoms are severe, causing complete nasal obstruction, uncontrolled nasal discharge, or even nose deformities, then the polyps will need to be removed surgically.

Surgical Treatment

A polypectomy may be carried out as an outpatient procedure to remove polyps that are closer to the opening of the nose.  The method is something similar to removing tonsils using a snare and is performed after applying a local anesthetic and a vasoconstrictor.  In the more recent times, though, powered instruments are utilized to conduct the polypectomy procedure, which give more accurate results and are less traumatic for the sufferer.

Another surgical method is the endoscopic sinus surgery that employs an endoscope for visualizing the nasal and sinus cavities to precisely locate the polyps.  Then powered endonasal instruments are passed to destroy the polyps.  This procedure is normally done to eliminate large polyps that do not shrink easily with corticosteroid use.
